Referrals and Admissions:

ABLE is proud to be a member of Wiltshire Council's Good Lives Alliance, working in partnership with local authorities, health professionals, and other providers to support people in achieving positive outcomes and fulfilling lives within their communities.

Referrals to our Registered Care Homes, King Street and Glanmor, are typically made through an individual's Social Worker or Community Psychiatric Nurse (CPN).

Referrals to our Supported Living Services, The Beeches and Long Street, can be made by an individual's Social Worker or Community Psychiatric Nurse (CPN), following an assessment of suitability and support needs.

All referrals are considered in accordance with ABLE's Referral and Admissions Policy to ensure that the service is able to meet the individual's needs while promoting a safe, compatible, and supportive living environment.

Please note: Funding approval must be confirmed and agreed by the relevant commissioning authority before a placement can be formally offered or secured.
